Cervical Smears 

Please note that it may now take 4 - 6 weeks for their result letter to arrive, not the 2 weeks that is quoted in the invitation letter.

This is due to workloads in the laboratories, this does not mean there is a problem with your sample.

Please do not contact the laboratory for outstanding results, or any other enquiry about test results – they cannot discuss results with you and will refer you back to their sample taker.

New Patient Registration

We welcome new patients who live within our practice area.

If you wish to register at Balderton Surgery you will need to provide proof of address. Photo ID is preferred e.g. Passport/Driving Licence but we can accept with either a bank statement or utility bill if you do not have a form of photo ID.

You will need to complete a GMS1 form along with a Medical Questionnaire and a sharing information consent form. The forms are available from Reception.

To ensure we can complete your registration when you come to the Practice, YOU MUST COMPLETE ALL of the forms and come into the surgery to provide the CORRECT PROOF of Identification (we will not accept Faxed or photo copied documents)

 We will only be able to complete your registration if all the above information is provided in person at the practice.
