All appointments made at Practice will be telephone appointment ONLY in the first instance.  The triaging clinician will advise on any further treatment required.


New Patient Registration

We welcome new patients who live within our practice area.

If you wish to register at Balderton Surgery you will need to provide proof of address. Photo ID is preferred e.g. Passport/Driving Licence but we can accept with either a bank statement or utility bill if you do not have a form of photo ID.

You will need to complete a GMS1 form along with a Medical Questionnaire and a sharing information consent form. The forms are available from Reception.

However, during the pandemic we have uploaded the forms needed to register with the practice below. These can be download and either be posted to us or they can be sent back to us via email:

To ensure we can complete your registration when you come to the Practice, YOU MUST COMPLETE ALL of the forms and come into the surgery to provide the CORRECT PROOF of Identification (we will not accept Faxed or photo copied documents)

 We will only be able to complete your registration if all the above information is provided in person at the practice.
